Please use your Alliance member ID card to help you fill in the following information. Enter your first and last name, date of birth and Member ID number exactly as they appear on your ID card. Do not include your middle initial. You will also need to enter your email address so we can verify your account.

Username
Username must be 8-15 characters long and can include uppercase letters, lowercase letters or numbers.
Password
Password must be 10 to 25 characters long and must include at least 1 uppercase letter, 1 lowercase letter, 1 number and 1 special character from the list: ( ) , . * + - / ! $ & ; ].

If you are an Alliance Medi-Cal member and are 12 years of age or older, you can create an account. Members under the age of 12 will need an authorized representative to create an account.
